All you need to do is use this form to request your rebate after completing your 4th trip.
Terms and Conditions
- Book and complete travel on 4 reservations between January 1st and December 31st, 2008 and receive a $50 Amazon.com Gift Certificate (subject to all Terms and Conditions from Amazon.com). All 4 trips must be consumed* by December 31st, 2008 unless this offer is extended. All claims must be made no later than January 31st, 2008.
* "Consumed" refers to the completion and payment of all travel. Cancelled reservations or "no-shows" do not count as consumed trips.
- Eligible reservations include and are limited to:
- Any online hotel-only reservation (but not those made by phone) with a total price including taxes and fees of at least $99
- Any Flight plus Hotel Vacation Package
- Any "Last Minute Deal" package
All reservations must be made via hotelmagician.com and you must record your confirmation numbers to apply for the rebate.
- After completing your 4th reservation, use this form to submit your claim. When all reservations have been verified as having been completed, an Amazon Gift Certificate will then be sent by US Mail or emailed in 4-6 weeks (or sooner). Regrettably, we cannot process any Gift Certificates until all 4 reservations have been completed due to the nature of reservations being cancellable until completed.
- Reservations made for back-to-back nights at the same hotel will NOT count as separate reservations for the purposes of this offer.
- This offer may be cancelled at any time. However, if the offer ends before December 31st, 2008, then all customers who have booked prior to such termination will still be entitled to a $10 Amazon Gift Certificate for each completed reservation (as long as they were booked before the offer termination date).
- If you make reservations on behalf of others, please use ONLY ONE EMAIL ADDRESS during this promotion.
- All reservations must have the same email address associated with them. Each confirmation number may be used only once. All 4 reservations submitted must have the same email address.
- Maximum of one redemption (for one $50 Amazon.com Gift Certificate) per person or household (verified using your name, email address, and mailing address) during the term of this promotion -- January 1 - December 31, 2008.
- This offer is open to residents of the USA and Canada only. Please note that the gift certificates will be issued at Amazon.com ($50USD) or Amazon.ca ($50CND) based on your country of residence.
